SMILE

The SMILE project is funded under the Sláintecare Integration Fund and will deliver a trial of the ProACT system to a new cohort of users. The focus of the SMILE project is to examine the potential of the ProACT platform to reduce unscheduled healthcare service use for adults with multiple chronic health conditions.
